Student Scholarship Form

The North Polk Community Schools Foundation Scholarship was established in 2004 to recognize well-rounded North Polk seniors who have contributed positively to their communities. The NPF Scholarship is a service-based scholarship and will be awarded based on a student's community service and volunteer work within their schools and communities. 

A total of $2,000 will be awarded this year to students planning to continue their education at an accredited post-secondary college, university, or vocational school.

Applications must be submitted by April 28. 

All applications are reviewed anonymously by the North Polk Foundation Scholarship committee. 
    In what activities do you participate? Please list all activities you have
    participated in during high school. Include the number of years, leadership roles,
    ​special committees or projects you have been involved with
    and any special recognition received.
    Have you been employed? If so, please list your places of employment.
    Include the number of months or years at the job, approximate number
    of hours worked per week and a description of your duties.
    Please list any community services or volunteer efforts. This can be with
    your church, any other group or on your own.
    How do you see yourself contributing to your community as an adult
    through volunteerism and community service?

    Additional Information

    Interview: After reviewing the applications members of the North Polk Community School Foundation  Board of Directors will select the top candidates for interviews.

    Award: The merits of the application and interview will be used as the final criteria to determine the scholarship winner. 

    Recognition: The Scholarship Recipient will be recognized at the Senior Night Awards Recognition Banquet.  

    Scholarship Payment: Scholarship winners will be asked to submit proof of their enrollment in a post-secondary educational program to the Foundation’s treasurer before payment will be made. Typically, this is simply a copy of an invoice, fee card, class schedule, or other document indicating the student’s enrollment at that institution. This is normally done in the fall following high school graduation, with payment being made shortly after that document is received.
